RF Microneedling

A class of energy-based device treatments that delivers radiofrequency energy through fine-gauge needle electrodes into the dermis, creating thermal coagulation zones that drive collagen and elastin remodeling. Three of the four globally distributed devices in this category are Korean-developed.

What it is

RF microneedling deploys an array of fine-gauge needle electrodes that penetrate the skin to a predetermined depth and discharge radiofrequency current within the dermal layer. The thermal zone is confined to the dermis while the overlying epidermis remains substantially intact.

Documented indications include skin laxity, rhytids, atrophic acne scarring, enlarged pores, and stretch marks. Select configurations also address vascular and pigmentary conditions.

How it works

RF energy in the 1 to 2 MHz range produces resistive (Joule) heating in tissue. Penetration is inversely proportional to frequency, with energy deposited preferentially in the higher-impedance reticular dermis.

All four named devices use bipolar configurations. Insulated needles (Morpheus8, Genius RF) confine current to the exposed needle tips, sparing the epidermis. Non-insulated or semi-insulated needles (Sylfirm X, Secret RF) distribute thermal effect through the dermis from the entry point.

RF discharge produces RF thermal zones (RFTZs), microscopic columns of coagulated collagen surrounded by intact tissue. The histologic response includes neutrophilic infiltration at 24 hours, fibroblast activation with TGF-beta and MMP upregulation, and collagen and elastin remodeling continuing 3 to 6 months.

Sylfirm X adds a dual-mode delivery system combining pulsed-wave RF (vasculature, melasma, post-inflammatory hyperpigmentation) with continuous-wave RF (collagen stimulation), at 2 MHz with a 25-needle tip and 0.3 to 4.0 mm depth range.

Origin and development

InMode Ltd. was founded in 2008 in Yokneam Illit, Israel and developed Morpheus8. Lutronic Corporation (Goyang, founded 1997) developed Genius RF with insulated needles and impedance sensing; Lutronic merged with Cynosure in January 2024 as Cynosure Lutronic, Inc. Ilooda Co., Ltd. (Suwon, founded 2006) manufactures Secret RF, with Cutera holding US distribution. Viol Co., Ltd. (Seongnam-si) developed both Sylfirm and Sylfirm X. BENEV launched Sylfirm X in the US as exclusive distributor in April 2021.

Post-procedure responses include erythema (24 to 72 hours, occasionally 5 to 7 days at higher energies), mild edema, pinpoint bleeding at needle entry sites, warmth, and skin flaking 2 to 5 days post-procedure.

Reported outcomes include improvement in skin firmness, reduction in fine lines and rhytids, improvement in atrophic acne scars, and improvement in pore appearance and skin texture. Sylfirm X pulsed-wave studies report reductions in melasma, post-inflammatory hyperpigmentation, and erythema or rosacea.

Less common responses include temporary hyperpigmentation in darker Fitzpatrick skin types at higher energy, prolonged erythema beyond seven days, infection, and rare scarring.

Korea vs US availability

Sylfirm X (실펌X) is heavily marketed across Korean booking platforms in 2025 to 2026. Per 강남언니 (Gangnam Unni) 2026 listings, 아미스킨의원 강남점/명동점 prices Sylfirm X at ₩220,000 per session, 와우성형외과 WOW 실펌X FULL케어 at ₩275,000, 리브의원 강남 실펌X at ₩242,000 (from ₩480,000), and 루비의원 명동 at ₩149,000.

여신티켓 (Yeoshin Ticket) platform guidance cites general RF microneedling at ₩300,000 to ₩800,000 per session for full face at non-promotional pricing.

US per-session pricing typically falls in $700 to $1,500 for platforms such as Morpheus8, with package discounts at 3 to 4 session bundles. Korean clients have access to the original Korean-developed devices at materially lower prices, and Korean clinics have built routine protocol fluency in pulsed-wave Sylfirm X applications for melasma that US clinics see less often.
