Hyaluronic Acid Fillers

Cross-linked hyaluronic acid gels injected into the dermis or subcutis to restore facial volume, soften wrinkles, and augment lip or facial contours. Korea's competitive domestic market makes brands such as Neuramis, Yvoire, and Elravie widely available at materially lower prices than imported equivalents.

What it is

Hyaluronic acid (HA) is a naturally occurring non-sulfated glycosaminoglycan that retains up to 1,000 times its weight in water, making it well-suited for soft tissue augmentation. Commercial HA fillers use bacterially fermented HA that is cross-linked, typically with 1,4-butanediol diglycidyl ether (BDDE), to resist enzymatic degradation and extend longevity to 6 to 24 months.

Fillers are classified by gel architecture. Monophasic fillers (Juvederm, Neuramis) use a homogeneous gel. Biphasic fillers (Restylane, Yvoire Classic) use NASHA particulate technology. Korea is a leading global HA filler exporter, with total export value exceeding USD 270 million in 2023, a 13 percent year-over-year increase per industry trade reporting.

How it works

Cross-linked HA gel restores volume through physical space occupancy. The viscoelastic hydrogel integrates into interstitial tissue and displaces soft tissue planes. Secondary mechanisms include hygroscopic volume amplification (binding interstitial water through osmotic forces) and receptor-mediated bioactivity via CD44, RHAMM, and TLR4 receptor interactions on fibroblasts and keratinocytes, which in lower-concentration or uncross-linked forms may stimulate collagen and elastin synthesis.

HA fillers are uniquely reversible by injection of hyaluronidase, which cleaves HA polymer chains. This enables emergency management of vascular occlusion complications and correction of overcorrection or migration.

Origin and development

Q-Medical AB (Sweden) developed the first cross-linked HA filler (Restylane) in 1996, with FDA approval in 2003. Allergan developed Juvederm, FDA-approved in 2006. LG Chem entered the Korean HA filler market in approximately 2006 to 2010 with Yvoire Classic, which earned a "World Best Product" designation from Korea's Minister of Trade, Industry and Energy in 2016 and now exports to over a dozen countries including Canada, Brazil, Mexico, and Argentina.

Medytox launched Neuramis around 2012 to 2013, using its proprietary High Concentration Cross-Linking technology. Neuramis has been exported to 35 countries as of 2024, including Ukraine following MFDS approval of Medytox's No. 3 factory in August 2024. Humedix (a Huons Group subsidiary) launched Elravie in 2012 and entered the US market via Amazon in 2026 with its Elravie Re2O ECM booster.

Commonly reported effects

Bruising, swelling, redness, pain, and tenderness are the most common reactions. The majority are self-limiting within 7 to 14 days.

The Tyndall effect, a blue-gray skin discoloration caused by light scattering off too-superficially placed filler (particularly in the tear trough), is treated with hyaluronidase and resolves within 24 hours in approximately 98 percent of cases.

Vascular occlusion is a rare but serious complication. A 2020 JAMA Dermatology study of 370 US board-certified dermatologists and 1.7 million syringes reported an incidence of 1 per 6,410 needle injections and 1 per 40,882 cannula injections. High-risk zones include the glabella, nasal dorsum, and nasolabial fold, where retrograde arterial embolism can cause irreversible vision loss. Hyaluronidase reversal is the standard emergency response.

Korea vs US availability

Per 모두닥 (Modoodoc) 2026 platform data, typical Seoul pricing ranges from approximately ₩99,000 to ₩175,000 per 1 cc for domestic Korean HA brands, and up to ₩290,000 per 1 cc for imported Juvederm or Restylane. The US average per the American Society of Plastic Surgeons 2023 fee data is approximately $715 per syringe.

Korean domestic HA brands are typically 50 to 60 percent cheaper than imported Juvederm at Korean clinics. International clients seek Korean-origin brands such as Neuramis and Yvoire because these products are not FDA-approved for US clinical sale, making Korea one of the few markets where they are accessible at scale, alongside the lower local pricing.
